Blended Case Management

• Mental Health Advocacy - Your assigned worker will help you get the best quality mental health care available.

• Treatment Monitoring - Your worker will ensure that treatment provided is being best utilized.

• In-home Visits - A Case Manager will visit you in your home to discuss your circumstances and help you come up with solutions.

• Case Management - We will coordinate all services provided, so that you don’t have conflicting appointments. Services provided through adulthood - You may be able to continue to receive services for your child through their 24th birthday, if needed.
