Community Based Services Department
Above all else, children desire to live with their families; and that is why we are committed to improving the family environment for the safety and security of all. While preventive services is an integral part of all the programs, Saint Dominic’s Home has focused an intensive effort in Rockland and in the Bronx, in particular the Fordham, University Heights and Mott Haven areas where, due to a great concentration of families who are economically poor, the need is great.


ASTAAN Prevention Program
(The Alternative Services to Avert the Abuse and Neglect)
Intended to prevent abuse, neglect and maltreatment of children and adolescents, our ASTAAN Prevention Program serves families with children from birth to 18 years. The program is designed to avert the need for children to be placed in foster care, thereby separating families and disturbing individual development. ASTAAN provides casework services such as crisis intervention, referral services, home visitation, casework counseling, escorting and advocacy. In addition, ASTAAN also offers specialized services unique to the program. Recognizing that change must occur to strengthen the home, our innovative Parent Education Training (PET) classes broaden and strengthen the families parenting skills. - The Bronx

Families First
(Home and Community Based Waiver Program)
Saint Dominic's Home provides a number of services designed to assist families in their efforts to remain united. Our Families First and Community Based Waiver Program help families who have children with serious emotional problems living at home. The Program reaches out to children ages 5-17 who are at risk for psychiatric hospitalization or re-hospitalization. Our services are tailored to the needs of each child. In partnering with families, there is an opportunity for them to choose both the services and providers they feel are best suited for their child. Saint Dominic's Families First Home and Community Based Waiver Program offers: Individualized care coordination; Strength Ðbased assessment and services; Home visitation; Family respite and support; Skill building activities; Intensive in-home services and 24-hour crisis response services.

Family Respite Program
The goal of the Family Respite Program is to help support and maintain family unity. By providing brief and temporary care for children in the homes of certified foster families, this program creates the opportunity to rebuild and restore family functioning over an appropriate period. In this way, children returning home are at considerably less risk for abuse, neglect or maltreatment.

The Family Respite Program offers:
Temporary respite care
Home visitation
Referral and advocacy services
Case management services.

Family Resolutions Program
The best solutions to family problems reside within the family. All families have inherent strengths, care about their children and have the ability and will to make changes necessary to their children’s benefit. The family Resolutions Program combines family group conferencing with child welfare case planning to expedite the return of a child to the biological family. Focusing on viable treatment plans that maintain child health and safety, avoid placement or expedite return home for children in foster care, the Program provides a short term service to families and provider agencies that have reached a barrier in planning. These dynamic partnerships empower families to participate in developing strategies that effectively prevent placement and facilitate a child’s return home. Family Resolution services include Family Group Conferencing, conference preparation services and post-conference follow-up. The Program works with TANF (Temporary Assistance for Needy Families) eligible families who have open preventive, protective, and/or foster care cases and is available to families within the Bronx community.

The CAFÉ
(Children and Family Empowered)
Serves seriously emotionally disturbed children as an alternative to residential care and their families. The Family Liaison receives referrals form the Rockland Single Point of Access for children's mental health services and meets with the family to a strength-based assessment. The liaison develops a treatment plan and collaborates with other community resources to provide services needed. The liaison acts and an advocate for the family with courts, schools and other community institutions.

Children First
(After School Program)
The program serves 15-20 seriously emotionally disturbed boys and girls ages 7-12. The Program is housed at Saint Dominic's School from 2:30 p.m. to 5:00 p.m. Monday through Thursday. The day is broken up into 3 different periods so that each child can be in a group of 6 or less and have the opportunity to spend time on the computer, spend time doing constructive activities in the gym and spend time having supervised homework.

Saturday Respite/Social Skills Program
Serves 20 seriously emotionally disturbed boys and girls ages 7-12. Groups are run in 9-week sessions twice a year. Children are picked up at their homes between 8:30 a.m. and 9:30 a.m. and are taken to Saint Dominic's School from 10:00 a.m. to 11:00 a.m. for social skills learning groups. They then go on a community activity that gives them the opportunity to practice what they have learned. They return home between 4:00 p.m. and 5:00 p.m. Transportation is provided.


Blended Case Management Program
Serves 44 children with serious emotional disturbances and their families. A team of case managers with shared caseloads provides services. The team consists of two Intensive Case Managers, each with a caseload of 12 and a Supportive Case Manager with a caseload of 20. The Blended Case Management program is also a step-down service for the Home and Community Based Waiver for children who require a lower level of care. The services assist children and their families to obtain needed medical, social, psychological, educational, financial and vocational services. The program provides 24 hour emergency coverage.
