The Individualized Care Coordinator (ICC) is responsible for engaging the child and family in a partnership of shared decision-making and service plan implementation throughout their enrollment in the HCBS Waiver. The ICC ensures and coordinates a comprehensive set of supports, resources and strategies for each child and family. The ICC works closely with outpatient clinics, day treatment programs and other providers to assure that Waiver services and clinical treatment modalities augment each other for optimal outcomes for children and families. The client to ICC ratio is 6 to 1. Must be bilingual (Spanish and English).

The Intensive-In Home and Crisis Response Worker provides ongoing intensive interventions aimed at de-escalating and educating the family in order to maintain family stability. The Intensive-In Home Worker works with the family based on time-limited goals based on the presenting crisis. OMH (ROCKLAND) MENTAL HEALTH PROGRAM LAKEWOOD
Responsible for the supervision of consumers in a community residence program serving Mentally Ill and/or Mentally Ill and Mentally Challenged individuals. Responsible for the safety of consumers, cleanliness of the residence, as well as providing and documenting restorative services based on accurate and consistent treatment plans which are reviewed quarterly. Attend staff development training. Sleeping on the job is prohibited.
On-Call
High school graduate. Experience in a residence dealing with Mentally Ill and/or Mentally Ill and Mentally Challenged individuals required. BA with experience preferred. Valid New York State driver’s license.
